Septal perforation repair, Rhinoplasty reconstructive, Pollybeak deformity, Nasal tip deprojection, Temporalis fascia graft

This lady had nasal septal surgery in the past and unfortunately collapsed her nasal tip giving her a polly beak deformity.  She also suffered from nasal crusting obstruction and on exam had a 2 cm mid septal perforation.  Temporalis fascia graft was harvested and grafted in between widely dissected nasal septal flaps.  The closure of the flap was watertight and without tension.  Some remnant septal cartilage was harvested and fashioned into a shield graft to provide tip support.  Postop photos show adequate tip projection and most importantly the hole has been closed.
